We believe coordinated care is the best care. When the social and clinical sides of service come together, our neighbors receive better help, faster. If you are a dedicated community member who shares that conviction, we need your voice at the table.
Our Board seats a maximum of 9 members, including the executive committee.
Our team grows through the neighbors we train. If you feel the call to walk with families in Angelina and Nacogdoches counties, the Cailan Care Coordination Academy is your next step.
